Please visit our events page or follow My Legacy's Facebook page for updates on where the subsequent sessions will be. Booking slots will be released 3 weeks before date of event.
The monthly mobile clinic sessions will take place at different heartland locations around Singapore to make it more convenient for all residents. -
We currently do not have a notification system to inform interested citizens when the appointment slots are open for booking. We will update our events page once booking is open

The number of slots available for the mobile clinic is dependent mainly on the availability of the pro bono LPA Certificate Issuers and ACP facilitators who are offering the services on weekends. Apart from that, it is also dependent on the space availability of the event

We do not accept walk-ins for the mobile clinic sessions. Limited slots are available based on the availability of our pro bono LPA Certificate Issuers (CI) and ACP facilitators. Booking a slot is required to ensure that we are able to cater sufficient CIs and facilitators for the demand

Individuals with CHAS Blue or Orange cards are likely to be of low income and will benefit most from the pro bono certification / facilitation sessions.

We encourage all Singaporeans to make their LPA and ACP early for peace of mind. Individuals aged 50 and above are at the stage where they are starting to plan for retirement and may have adult children who may be appointed as their Donees and Nominated Healthcare Spokespersons.
LPA and ACP are also particularly important for people who have chronic illnesses, early cognitive impairment, are frail, or approaching the end of life -

Do check with your preferred service provider on their charges. -
You can reschedule/cancel your booking via the email confirmation sent by Booking SG. As booking slots are limited, there is no guarantee that we can allow you to reschedule your appointment. Please email enquiries@mylegacy.gov.sg if you have any questions.

When you come down for your mobile clinic session, do remember to bring along your NRIC and CHAS Blue or Orange card for identification purposes.
For LPA, please complete your LPA form and ensure all your Donee(s) and Replacement Donee (if any) have accepted their appointment. You should have received an SMS/email notification to visit the certificate issuer. Your LPA status should be “Pending Certification”. Your Donee(s) do not have to be present.
For ACP, please complete and print out your ACP draft using the LPA-ACP Tool on My Legacy. Your Nominated Healthcare Spokesperson(s) have to be present.
